Every listing on Hospice for Choices comes from a public state licensing record. We do not accept payment for listings and we do not add ratings or reviews.

What we change
State records are published in capital letters. We convert names and addresses to normal capitalization and add map coordinates using the U.S. Census Bureau geocoder. We do not change license numbers, status or capacity.

Use the claim or correction form. Because license status and capacity come from the state, the licensing agency must also be informed of any change.
